Testing the proportional odds assumption

Cumulative logit model

How can I fill the entries in the grid of intervals and test?

The variable I added is categorical, and it is coded 0, 1 and 2.
I added a variable that in the equations windows shows coefficients β3,β4, β5, β6, β7, and β8.
In the interval and test window, the number of functions I should change to is 5, isn`t it?

But I don`t know how to fill the grid.
